Payment for goods and services is expected in full at time of the Funeral Arrangements.

The goods and services shown are those we can provide to our customers. You may choose only the items you desire. All though any cremation or burial arrangement you select will include a charge for our basic services and overhead.

CREMATORY SERVICE POLICY:

Veterinary Fees:

Forever Pets Funeral Home will not pay veterinary fees or charges.

Basic Professional Services of Crematory Director and Staff

This is the basic charge to each family we serve for our professional Services in assisting them. It includes, but not limited to a proportionate share of taxes, licenses, utilities and business expenses necessary to serve the public. Also included in this charge are the clerical and administrative services, preparation and filing of necessary permits required by the Illinois Companion Animal Cremation Act.

Companion Animal Care and Grooming:

All companion animals designated for private viewing will receive professional care and grooming.

Washing and disinfecting of companion animals remains:

Care of Sheltering remains.

Refrigeration of companion animal remains.

Services and use of Facilities:

The use of Chapel for visitation and services of crematory director and staff in supervising visitation are as follows:

Removal and Transfer of remains to crematory facility.

Utility vehicle and driver.
